- If Spanish is your preferred language, click the blue hyperlink to the left of the page, near the center. If English is your preferred language, skip to step 3.
- Enter the 18-digit code found at the bottom of your receipt. Click start to begin.
- You will be asked questions about your most recent visit. Please answer these as honestly and as thoroughly as possible. Answer all questions on one page before moving to the next page. Some questions will offer a text box to elaborate. Please use this whenever possible to give a better response.
